Use this form if you cannot not attend a scheduled final exam because of an unforeseen event such as an illness, death in immediate family, exam hardship (3 exams in less than 27 hours) or religious observance.
Further Information:
Supporting documentation is required. The rescheduled exam should be taken during the final exam period for that term.
